 GEAR Store Gift Card

Gift cards are only good for GEAR store purchases, in store or online at Gear by Monument Health. 

EMAIL ADDRESS REQUIRED for online purchases. An e-card with QR Code/Code will be emailed to you. A physical card(s) will also be sent via interoffice mail (or in-store pickup, if selected), with the matching QR Code/Code.

Gift Card(s) must be presented at the time of purchase, or scan QR code, or enter code to shop online. Monument Health GEAR is not responsible, for lost, stolen, damaged, or unauthorized use. Cards are nontransferable and cannot be redeemed for cash.
Expiration:  One year from purchase date.
